We look forward to your first visit and will help you in any way we can. To help us, we ask that you bring the following to your first appointment:
You will be asked to fill out a number of patient background forms, so please plan on arriving a few minutes early or you can download the forms here.
Medicaid Patients: You will need a referral from your primary care physician so that we can obtain a filing number.
Workmen's Compensation Patients: Your employer must call to make your appointment so that we can obtain the correct billing information.
Please schedule your appointment in advance. If you're unable to keep your appointment, please call us as far in advance as possible so we may reschedule that time slot. We sincerely respect the value of your time. However, surgeries and emergencies may cause unforeseen delays in our scheduling. You may call ahead to verify that the doctor is on time. If the doctor is not able to see you, the receptionist will reschedule your appointment for the earliest possible time.
If patients request copies of their medical records, we will be happy to supply them for a small fee. Copies of x-rays can also be provided.
