Our entire staff places the utmost importance on providing caring, professional service. You’ll see this service orientation in every one you deal with here, from the receptionists, insurance assistants, x-ray technologists, nurses, and therapists to the most senior physicians.

Your First Appointment

We look forward to your first visit and will help you in any way we can. To help
us, we ask that you bring the following to your first appointment:

A copy of your medical records and x-rays from any other doctor who has seen you for the same problem

A list of your current medications

A copy of your insurance card and driver’s license

A referral from your Primary Care Physician if required by your insurance carrier

You will be asked to fill out a number of patient background forms, so please plan on arriving a few minutes early or you can download the forms below.

Scheduling Appointments

To schedule an appointment with any of the doctors at our Dothan offices, please call (334) 793-2663 or (800) 460-2663, at our Enterprise office, call (334) 308-9797.

Medicaid Patients: You will need a referral from your primary care physician so that we can obtain a filing number.

Workmen’s Compensation Patients: Your employer must call to make your appointment so that we can obtain the correct billing information.

Please schedule your appointment in advance. If you’re unable to keep your appointment, please call us as far in advance as possible so we may reschedule that time slot. We sincerely respect the value of your time. However, surgeries and emergencies may cause unforeseen delays in our scheduling. You may call ahead to verify that the doctor is on time. If the doctor is not able to see you, the receptionist will reschedule your appointment for the earliest possible time.

Prescriptions and Renewals

Please request all prescriptions and authorizations for renewals during normal office hours when your records are available.

Medical Records Policy

If one of our physicians refers a patient to another doctor, we furnish that doctor with a copy of the patient’s medical records including any pertinent x-ray copies – at no cost to the patient.

If patients request copies of their medical records, we will be happy to supply them for a small fee. Copies of x-rays can also be provided.
